CTT
is a full support service model incorporating
case management and treatment for adults
with serious and persistent mental illness.
CTT is established as a primary, direct
service which provides comprehensive and
intensive outpatient mental health treatment,
rehabilitative and support services to those
persons for whom traditional community mental
health services alone have not been effective.
CTT services are for those persons with
serious mental illness who have not achieved
and maintained health and stability in the
community, and without theses services,
would continue to experience hospitalization,
incarceration, substance use/abuse, psychiatric
emergencies, and/or homelessness. Services
are guided by the principle that most individuals
with serious mental illness, can live successfully
in integrated community settings when adequate,
individualized supports and services are
provided. CTT merge clinical, rehabilitative
and support staff expertise within one service
delivery team that enables the individual
over time to develop the ability to access
and use other needed community services.
Service
Offering:
The program is designed to include the following
array of services:

Case management, including treatment
plan development and execution

Crisis assessment and intervention,
available 24 hours per day, 7 days per
week

Symptom assessment, management, psycho
education and individual supportive
therapy
-
Medication prescription, administration,
monitoring, and documentation

Referral to and coordination of substance
abuse services
-
Assessment and skill development in
the area of activities of daily living
-
Social, interpersonal relationship,
and leisure-time skill training
-
Family support and education
-
Coordination and advocacy, when indicated,
of support services, including health
care, housing, financial, transportation,
legal, educational and vocational need
