H.O.P.E.
Network Family Based Mental Health Services
are team delivered intensive therapy service
provided in the home and community to enable
families to care for their “at-risk”
child in the home and reduce the need for
out of home placement. Goals of the program
are strengthening the family unit, improving
coping skills, and serving as an advocate
for the child. To be eligible for Family
Based Mental Health Services, the following
medical necessity criteria must be met:
- Under
the age of 21
- Exhibits
serious emotional an behavioral disturbances
- Determined
to be high risk for out-of-home placement,
- Involved
with multiple system (i.e. CYS, Juvenile
Probation, Special Academic Needs, or
other Specialized community services
- Recommended
by a Psychiatrist, Licensed Psychologist,
or Primary Care Physician.

Family
Based Mental Health Services is a team-delivered

community. Services are child-centered and
child-focused while service planning and
delivery are family driven. Family Based
accomplishes goals by integrating mental
health treatment, family and community supports
and case management.
Family
Based therapists provide crisis intervention
24 hours a day for H.O.P.E. clients.
