Mobile medication nursing services are nursing services available to Chestnut Ridge Counseling Services, Inc. consumers age 18 or older, seen by an agency psychiatrist and maintained on medication(s). This service will assist in facilitating coordination of medication services for consumers in an effort to maintain or improve their overall functioning in the community.

Upon referral by an agency psychiatrist, the nurse provides:

• Nursing assessments for the purpose of monitoring medication and its effectiveness or other issues identified by the psychiatrist.
• Ongoing consumer education regarding the medication regime.
• Assistance with medication set-up that may include setting up pill containers. The goal of this service is to assist the client to develop the skills needed to manage medication(s) independently.
• Provide consultation and education to both the consumer and identified family members or care givers with the goal to be focused on overall wellness promotion.
• The nurse can assist with the coordination or referral to support services available in the community such as case managers or transportation.

Throughout the service provision, the Mobile Medication Nurse provides the treating psychiatrist with feedback regarding the consumer’s progress while in services.
