What is School Based Therapy?

School Based outpatient therapy is mental health therapy provided at school during the course of the student’s day. The student is referred to a school based therapist who will see the student during the school day for individual therapy for identified behavioral health problems.

Therapy can be provided in the school setting while maintaining the same confidentiality standards of traditional outpatient therapists.

Referrals for school based services can be made by school personnel, the SAP team, the student, other CRCSI programs, and/or a family member.

Referral Process

The referral form is available below. If you have Adobe Acrobat Reader it can be filled out on your computer then printed, otherwise print the form and fill out in pen. Once filled out, the form can be faxed to (724) 439 - 8487, mailed to, or taken to any of our locations. For security reasons do not send these forms over email.
