About The Program

Chestnut Ridge’s Peer Support Program instills hope in individuals that recovery is not only possible, but probable. With the assistance of our Certified Peer Specialists, you can achieve the goals you established. This program is centered on the recovery goals identified by you and incorporated into your Individual Service Plan following the completion of a strength-based assessment.

You have the opportunity to participate in and make decisions pertaining to your services. The philosophy of peer support is that services are person-centered with a recovery focus and is a tool to facilitate the development of self-help skills. Individual advocacy, crisis management support and skills training are some of the services included.

How Do Referrals Work?

Peer Support Services serves consenting individuals 14 tears of age and older who have a mental illness or a serious emotional disorder regardless of gender, race, sex, religion, ethnic origin, economic status or sexual orientation. Anyone can make a referral for services through the form below. . Consumers can self-refer or request that a referral application be completed by a member of their treatment team. The referral form is available below. If you have Adobe Acrobat Reader you can fill it out on your computer then print it; otherwise, print the form first and then fill it out in pen. Once filled out, you can fax it to (724) 439 - 8487, mail it , or bring it to any of our locations. For security reasons do not send any forms over email.
